ABI PROGRAM SUPERVISOR
Cheshire and Hartford
·         Qualified candidates will coordinate all aspects of the DSS and DMHAS Acquired Brain Injury program for consumers   enrolled in residential or day programs.
·         Assist with the recruitment of new consumers, help develop hospital discharge plans and facilitates transition into the community-based program.
·         Assist consumers and staff in accessing all resources and benefits available including financial, counseling and housing programs.
·         Develop and monitor ISPs and behavioral plans for all ABI participates in assigned programs.  Coordinates and communicates regularly with team members including other providers.
·         Oversee and ensure compliance with HIPAA, CARF and all other relevant federal, state and local regulation.

Job Requirements:
·         Bachelor's degree or equivalent education or training in psychology, human services, behavioral health, acquired brain injury.
·         2yrs. experience in direct care, 1yr experience in case management, supervisory experience preferred
·         Excellent organizational, administrative, verbal and written skills
·         Proficient computer skills  
·         Valid CT license and daily use of a registered and insured vehicle
·         Drug screen and background check required

Community Experience Program Assistant

We are seeking an individual to support our clients who are working in programs that enhance their community integration and vocational skills.
 
Job Requirements:
 
·         Combination of education, training and/or practical work experience in Human Service environment. 
·         One-year practical experience working with people who have severe physical and/or mental disabilities.
·         Current and valid CT driver's license and eligibility for public service license is required.
·         Ability to physically assist consumers with lifting and moving.
·         Ability to operate a Hoyer lift.
·         Ability to lift up to 50 pounds.

  • Bachelor's degree in Human Services, Special Education, or related field, or Associate's degree with two-year practical work experience or professional certification.
  • Experience working with adults with behavioral health conditions, preferably in vocational programs.
  • Good organizational, communication and interpersonal skills and ability to work as a team.
  • Bi-lingual (English/Spanish) is a plus.

Community Living Support Staff
Bridgeport, Central CT, Manchester
Qualified candidates will assist people with acquired brain injuries or other disabilities to integrate back into the community. Residential aid associated with daily living and socialization skills. Availability to work flexible hours including nights and weekends is a plus. Position requires high school diploma or GED equivalent plus 1 year experience working with individuals with disabilities. Must have a valid CT driver's license and reliable, insured car.  Goodwill provides disability related training and First Aid and CPR. Must be able to keep timely and accurate notes on an electronic database.  Drug screen and criminal background check required.
